Madison County Youth Football Injury Liability Waiver

I, the parent or legal guardian of the participant, hereby grant my permission for him/her to participate fully in any activities, practices and games of said activity. Furthermore, I hereby give my permission to take said participant to a doctor or hospital and hereby authorize medical treatment in my absence if necessary.

In consideration for being accepted by Madison County Youth Football for participation in this activity I hereby release forever discharge and agree to hold harmless Madison County Youth Football and the directors thereof any and all liability claims or demands for personal injury, sickness or death, damages and expenses of any nature whatsoever which may be incurred by your child while participating in the above-described activities.

Furthermore, I hereby assume all risk of personal injury, sickness, death, damage, expense as a result of participation in recreation and activities involved therein. Further authorization and permission are hereby granted to furnish transportation or call for transportation if such is needed.

The undersigned further hereby agree to hold harmless and indemnify said organization and it’s directors, employees and volunteers or any liability sustained by said organization as the result of negligent willful or intentional acts of said participant, including expenses incurred attendant thereto.

Further, should it be necessary for the participant to return home due to medical reasons, disciplinary action or otherwise, I will arrange the necessary transportation immediately after notification of such action or otherwise I will arrange the necessary transportation immediately after notification of such.

 

Madison County Youth Football Photo/Video Release

I hereby give permission for images of my child, captured during the Madison County Youth Football season through video, photo and digital camera, to be used solely for the purposes of Madison County Youth Football promotional material and publications, including website entries, and waive any rights of compensation or ownership thereto. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein a likeness appears.
